Vesta, Inc.
provides comprehensive behavioral healthcare to include clinic, day program, and residential homes for those suffering from mental health disorders as well as transitional housing for homeless veterans.
Vesta, Inc is currently recruiting for the position of Case Manager for our Lanham Region's PRP program.
This position will provide coordination for consumers will provide support through coordination of services, outreach, and sourcing additional resources for social, health, and medical needs.
This position requires a great deal of hands on, on call, care to ensure clients achieve goals, reduce somatic health concerns, and are receiving all necessary support.
COVID-19 Vaccine Mandate:

This position is a Full-Time, exempt position with availability of Monday - Friday 8am-4:
30pm.
Should also be available nights and weekends as operations or client issues require.
Requirements:
Graduate Degree in the Health and Human Service field or Bachelor's Degree with experience in a comparable role.
Have ability to research services and benefits and link consumers based on needs and treatment goals.
Have experience in a coordinator or supervisory role in a residential/group home setting.
Have knowledge of mental health disordersHave 3 years of experience with case managementPossess a valid driver license, reliable transportation, and willingness to transport clients in personnel vehicle as well as drive a 15 passenger van.
Ability to be flexible and available to respond to consumers and crises situationsPossess excellent time management and organization skillsHave the ability to work independently and handle high stress situationsHave excellent verbal and written communication skillsOperate in the highest ethical standards and comply with all regulatory requirements
Responsibilities:
Provide supportive counseling and advocacy for clients.
Assist with the completion of client assessments providing environmental, psycho-social, financial, employment, housing, educational, and health information as appropriate.
Assist clients in locating educational/job training opportunities.
Maintain complete and accurate case records completing all documentation in a timely manner in accordance to policy and procedure as well as regulatory requirements.
Complete all necessary administrative tasks for case management, including maintenance and follow-up with all paperwork, phone calls and emails whole maintaining accurate and complete case records.
Refer clients to appropriate services to include health care providers, educational support groups, and family events.
Conduct bi-weekly house inspections to ensure they meet licensing standards, including but not excluded to the monitoring of food levels, MAR, medications; report and address concerns.
Assist with the distribution of weekly and monthly allowances and collect fees from clients in a timely manner.
Conduct on and off site meetings and groups with clients as required.
Review doctor referrals and somatic care forms; assist with consumer follow up; act as liaison and contact for clients hospitalized.
Ensure that homes meet the standards defined for comfort, cleanliness, and safety.
Report and follow up as needed on needed repairs.
Ensure adequate supplies (emergency food, food, medicine, first aid, etc) are available in quantities as defined by the organization.
Conduct fire drills.
Salary at time of hire ranges from (depending on degree and experience):
Bachelors Degree:
$45,430 - $50,881.
60Masters Degree:

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
